I've noticed that some of the paragraph comments on my work are missing. I get the notification and the comment appears in my inbox, however when I try to reply I can't find it.
I thought it might be because the readers are deleting them, but is that really it?
Is anyone else facing the same problem?

Well i did, and it was just them deleting them not a bug as far as i know, and they won't disappear from the notification feed so just ignore them.
Correct, this is just the user deleting the comment. They can't edit comments, so they will just delete it and post another, or if they have second thoughts, just delete it.
